Serving Kings Langley and surrounding Blacktown and Hills District areas

1. Our services

We provide chiropractic assessment and care for musculoskeletal conditions. Andrew Lyell is a chiropractor registered with the Chiropractic Board of Australia. Your care will be provided by an appropriately registered practitioner.

These terms apply to our services and appointments. They do not replace the information provided during your consultation, your informed consent to a particular treatment, or our Privacy Policy.

2. Appointments and cancellations

Please contact the Practice if you need to change or cancel an appointment.

If you cancel with less than 24 hours’ notice, or do not attend your appointment, a cancellation fee of 50% of the appointment fee may be charged. We will confirm this policy with you when you book. We will not charge a fee that has not been disclosed to you in advance.

3. Fees and payment

We will tell you the applicable fees before providing a service. Payment is due at the time of your appointment unless we have agreed otherwise.

HICAPS may be available for eligible private health fund claims. Any rebate or claim is subject to your fund’s rules and your level of cover. You are responsible for any gap or amount not paid by your fund.

Medicare benefits for eligible chronic condition management services depend on current Medicare requirements, including referral and eligibility rules. Workers compensation and CTP services are subject to the relevant insurer’s approval and requirements. DVA services are subject to current DVA referral and approval requirements.

Unless we have agreed otherwise in writing, you are responsible for fees not covered or paid by Medicare, a health fund, insurer or DVA, including where a claim is declined.

4. Assessment, consent and treatment

Your first appointment generally includes a health history and clinical assessment. Your chiropractor will discuss relevant findings and care options with you.

Before treatment, your chiropractor will explain the proposed care and any material risks relevant to you, and obtain your informed consent. You may ask questions, decline a proposed treatment or withdraw your consent and stop treatment at any time. We will discuss reasonable alternatives where appropriate.

Please provide complete and accurate information about your health, symptoms, medical history, medications and relevant changes in your condition. This helps your chiropractor assess what care may be suitable. If you choose not to provide relevant information, it may affect the care we can safely provide.

5. Results and clinical assessment

Chiropractic care may assist with certain musculoskeletal conditions, but results vary between individuals. We do not guarantee a particular outcome.

Treatment is subject to individual clinical assessment. Your chiropractor may recommend other care or referral to another health practitioner where appropriate.

6. Health information and privacy

We collect health information that is reasonably necessary to assess and provide care, manage appointments, process payments or claims, and meet legal obligations. Health information is sensitive information.

We handle your personal and health information in accordance with applicable privacy laws, including the Health Records and Information Privacy Act 2002 (NSW) and the Privacy Act 1988 (Cth).

We may use your information for your care and related practice administration. We may share relevant information with another treating health practitioner where you consent, or where disclosure is otherwise permitted or required by law. We will explain relevant disclosures where appropriate.

You may ask to access or correct information we hold about you, subject to applicable law. Our Privacy Policy explains our information-handling practices, including how to make a privacy complaint. It is available on our website or from the Practice on request.

If you do not provide information reasonably needed for your care, this may limit our ability to assess or treat you.

7. Website information

Information on our website is general information only. It is not medical advice and is not a substitute for an individual clinical assessment. If you have a health concern, contact a health practitioner. In an emergency, call 000.

8. Patient conduct and safety

We ask patients to provide accurate information, follow reasonable safety instructions and treat our staff and other patients respectfully.

We may pause or end an appointment if conduct creates a safety risk or prevents care from being provided appropriately. Any fees already incurred remain subject to applicable law and any prior agreement.

9. Complaints

If you have a concern about our services, please contact the Practice so we can try to resolve it.

You may also contact the NSW Health Care Complaints Commission (HCCC) or the Health Professional Councils Authority, which includes the Chiropractic Council of New South Wales. You may contact the NSW Information and Privacy Commission about a health privacy complaint.

10. Consumer rights and liability

Nothing in these terms excludes, restricts or modifies any right, guarantee or remedy that cannot lawfully be excluded, including your rights under the Australian Consumer Law.

To the extent permitted by law, our liability is limited to the remedies available under applicable law. Nothing in these terms limits liability where it would be unlawful to do so.

11. Changes to these terms

We may update these terms from time to time. Updated terms will be published on our website and will apply to services booked after the update takes effect. A change will not affect rights or obligations that have already accrued.
